Social Prescribing Manager

Hours: Full time 37 hours per week. Permanent subject to funding.

Social Prescribers work closely with Primary Care Teams to enable individuals to find the support they need in their community to manage their health and wellbeing. Social prescribers give people time to talk about what “matters to me” and support them to unpick complex issues affecting health. They enable people to have more control over their lives, develop skills and give their time to others through involvement in community groups. Social Prescribers will see people in their local GP practice, community venues and may visit people in their homes.

As Social Prescribing Manager, you will be responsible for the operational management of the team providing coaching, advice and guidance to staff, ensuring that standards and targets are met consistently and developing partnerships with GPs and other stakeholders. You will carry a small caseload.

What we offer: In addition to a salary of £27,973 per annum, we offer:

  • 25 days leave per year, rising to 30 days per year, plus bank holidays (pro rata)
  • Contributory Pension Scheme

Social Prescribing Team Leader

Hours: Full time 37 hours per week. We will consider requests for flexible working, including part-time and flexible hours. Permanent subject to funding.

Social Prescribers work closely with Primary Care Teams to enable individuals to find the support they need in their community to manage their health and wellbeing. They enable people to have more control over their lives, develop skills and give their time to others through involvement in community groups. We are looking for an experienced, passionate individual with experience of working with people people with health and social needs, or community-based work with socially excluded groups, with the ability to understand diverse and complex client needs and to use a ‘brokerage’ approach to meet these needs, and can lead a team of dedicated Social Prescribing Link Workers to deliver a quality service.

What we offer: In addition to a salary of £25,368 per annum, we offer:

  • 25 days leave per year, rising to 30 days per year, plus bank holidays (pro rata)

Lead Youth Worker

Hours: Full time 37 hours per week. Permanent subject to funding.

The main purpose of this role is to support our youth work team to deliver high quality, professional
youth work. You will provide support and systems so that you can be confident that all youth work is
helping young people to develop the skills, confidence and resilience they need for their future lives. You
will also deliver a minimum of 2 sessions per week.

South Gloucestershire as the local authority commission out its youth work provision and Southern
Brooks Community Partnerships are the lead partner for lot 1. Until now there has been a mix of local
authority and the VCSE sector delivery youth provision across the area. This new method of working will
help provide a consistent approach across the area. We are also part of another youth work partnership
who work together to secure finding and provision for youth work across our areas.

Southern Brooks Community Partnerships leads a partnership which delivers youth work across
Patchway, Filton, Bradley Stoke, Stoke Gifford and Thornbury. We are working closely with Creative
Youth Network and Learning Partnership West who lead in other areas of South Gloucestershire.
Together, we will ensure that young people are able to access high quality youth work that is driven by
their needs and is embedded within the local community.

This is an exciting opportunity to help shape our future youth provision in South Gloucestershire and to
develop our young people to become future leaders

Assistant Youth Worker

Hours: Minimum 3 hours per week. Permanent subject to funding.

The main purpose of this role is to deliver fun, interactive youth work sessions to young people. You will provide support to help young people to develop the skills, confidence and resilience they need for their future lives.
