One You South Gloucestershire brings everything together to support local people in South Gloucestershire to improve their wellbeing as well as make healthier lifestyle choices.

Aims of the Service

• Feel more confident about managing your emotional and mental wellbeing
• Feel more able to get involved in your local communities
• Feel less isolated, and less anxious
• Feel able to make changes in other areas of your life

One You South Gloucestershire is for all adults over the age of 18 who are South Gloucestershire residents or registered with a GP practice in South Gloucestershire.

1-1 Support

Who can refer for Wellbeing support?

Referrals can be made by any service, on behalf of an individual, as long they meet the criteria listed above and consent is gained to do so. 

Self-referrals are also accepted. 

Who can be referred? 

To be eligible for support from One You South Gloucestershire Wellbeing, an individual must meet the following criteria: 

  • Over the age of 18 
  • And either:
    • Living in South Gloucestershire or registered at a GP surgery in South Gloucestershire, or  
    • A carer living in Bristol, caring for someone who lives in / is registered at a GP surgery in South Gloucestershire. 

Who may not be suitable for OYSG Wellbeing? (Exclusion Criteria) 

  • People who do not fall into one of the above groups. 
  • People who are currently experiencing a mental health crisis or needing urgent / emergency support. OYSG Wellbeing is not an emergency service and are unable to support in this situation – we aim to make initial contact within 3 working days, to then arrange an initial appointment with an individual. 
  • People with severe and enduring mental health conditions. Wellbeing Coaches are not qualified mental health professionals. 
  • People that are well known to secondary care mental health service or currently requiring / receiving secondary care mental health input. 
  • People who do not want to engage with the Wellbeing service. Consent is required for referral on someone’s behalf. 
  • People who need support to manage alcohol or drug addiction.  A Wellbeing Coach can work with them to address other appropriate issues, but only if they are willing and able to engage – they cannot work with people while they are under the influence of drugs or alcohol. 

What can a Wellbeing Coach help with? 

The Wellbeing Coach is there to listen to the individual and help to identify or implement lifestyle changes, such as:

  • Looking after themselves 
  • Managing symptoms 
  • Work, volunteering and other activities 
  • Finding legal advice or debt counselling 
  • Information and guidance on what local resources there are  
  • Feeling positive, improving confidence and self-esteem 
  • Carers

How can I make a referral? (Both self-referrals & from another professional)

Self-referrals can be made using this form.

What will a Wellbeing Coach do once a referral has been made? 

The client will be allocated to a Wellbeing Coach, who will make initial contact with them within 3 working days of the referral to arrange a 30-minute behaviour change phone call. 

During the first 30-minute phone call, the Wellbeing Coach will - 

  • make a wellbeing plan with the client about what small steps they can take to improve their wellbeing 
  • give them information about our sessions
  • give signposting to other services if needed 
  • make an appointment for the next phone call 

2 or 3 follow-up phone calls will be offered, to help motivate the client with their wellbeing plan. These can be weekly, fortnightly, or monthly, depending on the needs of the client. 

What if a Wellbeing Coach is unable to make contact / follow up with a participant? 

  1. Should a Wellbeing Coach be unsuccessful in making contact, a text message / voicemail will be left to say what time / day the next attempt will be. We will also encourage the participant to respond with a time that suits them better if needed.
  2. If a Wellbeing Coach is unsuccessful on the second attempt, a further text message / voicemail is left stating they will have 1 more attempt at making contact and will presume support isn’t required if they are unable to contact. 
  3. After a 3rd unsuccessful attempt, they will leave this participants referral open for 7 days. If no response is received in these 7 days, the case is closed. 

This will also be the case for face-to-face appointments, but the case will be closed after 2 attempts if they do not attend.  

OYSG Wellbeing Sessions & Groups

What are Wellbeing Sessions & Groups? 

OYSG Wellbeing also provide a wide range of sessions and groups that are open for individuals to sign up. We offer both online* and face-to-face groups and will continue to provide this moving forward.  

These sessions look to embody 1 or more of the 5 Ways to Wellbeing. 

The team look to provide different types of sessions which are reviewed regularly, dependent on interest and feedback, which focus on at least 1 of the 5 ways below to improve wellbeing.

Is there a cost? 

People can register for a session or group for free.
