Social Prescribing Link Worker - Southmead Hospital

The purpose of this role is supporting patients who are leaving hospital and assisting them in settling back into daily life and their community.

Link Workers work alongside hospital teams, so individuals are discharged safely and find the ongoing support they need. An important part of the role is giving people time to talk about what matters to them and guiding them to unpick complex issues affecting their wellbeing. Link Workers enable people to have more control over their lives, maintain independence, and reconnect with their community. The work is based in the hospital South Gloucestershire Social Care team with requirement for home and community visits.

Salary and Hours: £25,079 per year. 37 hours per week (full time)
