Welcome Home Worker

Southern Brooks is looking for a new Welcome Home Team Worker to join a brand-new project, working in conjunction with Age UK South Gloucestershire with the aim of providing short to medium support to people who have been discharged from hospital to stay well in their own homes.

This is an exciting new project which will link in with our established Hospital Link Workers in providing the best possible support for patients who have been discharged from hospital and assist them in settling back into daily life and their community.

If you were already in this in this Welcome Home Worker role, you might be doing some of these things this week:

  • Carry out home assessments to co-produce client action plans with the client to follow.
  • Support individuals to address barriers to services or maintaining independence.
  • Accompany individuals to community groups, activities, and statutory services.
  • Work closely and establish effective working relationships with other professionals (hospital discharge teams, occupational therapists & other professionals) to ensure joined up support to clients.

To apply, you will need at least 2 years’ experience working with people with health and social needs, or community-based work using a person-centred approach.

As a vital member of our team and in return for your expertise, empathy and passion for supporting adults to live well in their own homes, we will provide a favourable salary of £25,580, excellent annual leave and sickness benefits, and will even give you the day off for your birthday! And you will join a passionate, inclusive, committed and welcoming workplace.

Apply and register your interest in this Welcome Home Team Worker position in South Gloucestershire by 9am on Friday 1st December 2023.

Interviews will be held online on 12th December 2023.

Please send all enquiries to: recruitment@southernbrooks.org.uk

We look forward to hearing from you!
