What do we do with your information?

Southern Brooks Wellbeing Project - Privacy Notice

Southern Brooks Wellbeing Project services are usually based in the community and we deliver our service from GP surgeries, sports centres and other community venues. However, we are currently providing our services online and over the phone due to covid restrictions. 

We work with people over the age of 18 providing advice and support to help people improve their emotional and mental wellbeing. 

What will we do with your information?

To access the wellbeing service, you can contact Southern Brooks directly to ask about this project and the services you can access, register for events via Eventbrite, complete a One You South Gloucestershire service request form, phoned One You South Gloucestershire and asked about services, or you can ask another professional working with you to contact our service on your behalf. 

All of these different ways of asking for help from Southern Brooks Wellbeing service require you to give your consent for your information to be shared.  This may be via an online booking form, an online information form, in person or over the phone to another person who referred you.  

The information that has been shared about you includes: your full name, gender, date of birth, ethnicity, address, contact details (telephone number and/or email).   

We will use your personal information to provide you with a service. We will only share your personal information with our partners who have referred you, or with those that are involved in working with you, but no one else without your permission. Anonymised data may be shared with One You South Gloucestershire and The Department of Health. The information we share will help to understand how we have helped you. 

Upon completion of the service, your personal information will be kept for seven years. Statistical information about people who use the service may be retained for longer than this, but this information would not identify you in any way. 

Data storage and retention 

This information will be entered onto our secure data system Charity Log.
