What do we do?
We deliver our service in hospital to patients aged 18+ who live in South Gloucestershire, visiting patients on their wards.
We connect patients with community services when they no longer need to stay in hospital. Working directly with patients and their families, we link them to services they need and want. We support people to achieve their goals to return home quickly, safely and engage with their communities.
We signpost and refer to a range of services providing:
- Benefits support and financial advice
- Personal alarms and key safes
- Carers support
- Home adaptations
- Adult social care
- Local activities which support health and wellbeing
For patients who have returned home:
We make follow up calls to patients to see how they are settling in at home.
