What do we do?

Using person-centered care, we connect patients aged 18+ in the hospital with other voluntary and community services to enhance mental health and wellbeing, with the aim of getting people home more quickly and reducing readmissions.

What can we support with?

  • Navigating to services that can reduce social isolation and improve wellbeing
  • Support for carers including identification of services
  • Signposting, and support to access, benefits services
  • Promotion of, and referrals to, green prescribing, physical activity and exercise services
  • Link to community groups

What can't we support with?

  • Physically supporting patients with care and rehab
  • Time-specific visits to patients
  • Clinical applications
  • Formal social services support
