Social prescribing is a national NHS England programme. Social prescribing link workers give people time, focusing on ‘what matters to me’ and take a holistic approach to people’s health and wellbeing. They help facilitate goals by linking people up to organisations, community groups and services.
Southern Brooks has been commissioned to deliver support within several GP Practices in South Gloucestershire. Our link workers have been working with patients to improve their wellbeing since 2020.
We work with GPs and other health professionals to offer support to those who could benefit from social prescribing. Referrals are made by health professionals into the service.
Our service is delivered through short-term and brief interventions. Typically, this would be 4-6 sessions over the phone, though we are able to provide face to face appointments. We are able to support with:
- Low to moderate mental health conditions including anxiety, depression, sleep issues and low mood
- Poor wellbeing such as loneliness and isolation
- Poor physical health requiring behaviour changes and support
- Support for those with long term conditions that affect their mental wellbeing
- Bereavement support or accessing support for domestic/sexual abuse.
- Housing, finances, employment and other social issues affecting wellbeing
