1-1 Support

Who can refer for Wellbeing support?

You can refer yourself, or you can ask a professional from any service to refer you, if you meet the criteria below.

Who can be referred?

To be eligible for support from One You South Gloucestershire Wellbeing, you should be:

  • Over the age of 18, and either -
    • Living in South Gloucestershire or registered at a GP surgery in South Gloucestershire, or
    • A carer living in Bristol, caring for someone who lives in or is registered at a GP surgery in South Gloucestershire.

Is OYSG Wellbeing right for me?

Our Wellbeing Coaches are not qualified mental health professionals, so we are not able to support people who are currently:

  • experiencing severe and enduring mental health conditions that aren’t currently well-managed
  • in a mental health crisis or in need of urgent or emergency support
  • well known to secondary care mental health service or currently requiring / receiving secondary care mental health input
  • under the influence of drugs or alcohol in a way that makes it difficult to work with a Wellbeing Coach

If you need mental health support, please call the 24/7 Support and Connect Helpline on 0800 0126549. This is a confidential helpline that is open all hours, providing immediate emotional and practical support. It is staffed by experienced counsellors who will offer emotional support and who can connect you to other organisations available in Bristol, North Somerset, and South Gloucestershire.

If you need support with drug or alcohol dependency, please contact DHI (Developing Health & Independence) on 0800 073 3011, 01454 868750 or info@dhisouthglos.org.uk.

What can a Wellbeing Coach help with?

The Wellbeing Coach is there to listen to you and help you identify or make lifestyle changes by

  • Looking after yourself
  • Managing your symptoms
  • Finding work, volunteering or other activities
  • Finding legal advice or debt counselling
  • Finding out what support is available to you
  • Telling you about what you can access in your community
  • Helping you to feel positive, and improve your confidence and self-esteem

How can I make a referral? (Both self-referrals & from another professional)

If you have any questions about the service, please call (01454) 868583 or email wellbeing@southernbrooks.org.uk.

Self-referrals can be made using this form.

If you would like a professional to refer you, they can use this form.

What will a Wellbeing Coach do once a referral has been made?

A Wellbeing Coach will get in contact with you within 3 working days of the referral to arrange a 30-minute behaviour change phone call.

During the first 30-minute phone call, the Wellbeing Coach will -

  • make a wellbeing plan with you about what small steps you can take to improve your wellbeing
  • give you information about our sessions / workshops
  • give information about other services if needed
  • make an appointment for the next phone call

2 or 3 follow-up phone calls will be offered, to help motivate you with your wellbeing plan.

What if a Wellbeing Coach is unable to make contact?

If your Wellbeing Coach doesn’t hear from you, a text message / voicemail will be left to say what time / day the next attempt will be, and to ask you to let them know if there’s a more convenient time for you.

If your Wellbeing Coach doesn’t reach you on the second attempt, they’ll send another text message / voicemail saying they will make 1 more attempt at making contact and will think that you don’t need/want support if they don’t hear from you.

After a 3rd unsuccessful attempt, they close your case if they don’t hear from you within 7 days.

For face-to-face support your case will be closed if you miss 2 appointments.
